It should have a bit of gameplay on Earth while the fact that you're from Earth should bring mechanics to your gameplay.Ultima, one of the oldest RPGs that helped spawn the JRPG genre is an isekai. The hero and several of his companions are from Earth, but the games take place in another world called Britannia.
That would be the maxim, but if we're talking purely story, we have a few isekai, just not enough to make it some kind of a dealbreaker for new games.
The same way everyone creates their own little fantasy world.also: how would you make an isekai video game? not one based on an existing IP like an anime/manga/LN but a fresh IP
